Place your jack about mid-way under the driver seat.  Using a scrap piece of 2x4 about 6-8" long as a buffer (in retrospect, I'd use pressure treated lumber scraps), GENTLY raise the driver side of the body - this will take more travel on the jack than you expect, since you have to account for the suspension opening up from the loss of the weight of the body.

Once you get enough room and put your spacers in their proper places, drop all three bolts into place, re-using the stock washers, and making sure the bolts pass through the stock body bushings.

Gently lower the body back on to the frame.  The middle body mount may not compress as much with the new spacer in place (apparently, Toyota somehow attaches the large lower washer to the bushing, so when you put the body back down, the bushing will appear to have a gap).  If the new bolts are not long enough to get the nut back on with the washers in place (as happened to us), re-use your old hardware.  Tighten the bolts finger tight, as you will have to re-align the cab to the bed before you are finished, not to mention that you need the driver side loose to get the passenger side done.

Simply reverse the side to get the passenger side done.

Once you have both sides done, GENTLY 'hip' the body into proper alignment.  To measure if you are centered, use a yardstick along one of the bends along the middle of the body to not only measure how far the cab is from the bed, but how much clearance you have left-to-right (the clearance should be equal - ours was about 1/16".
