Ok, the same plan applies to the rear of your truck.  Before beginning, remove the gas cap and the black plastic cup around the gas tank hose.  Again, start with loosening all the bolts, then remove the driver side bolts, saving the washers - Note: you will not be able to save the rear washers, they are part of the bolt.  Get new ones at either your local Toyota dealer or a hardware store.  The bed is light enough that one of you can lift it while the other places all three spacers.  

Perform the same procedure on the other side, check the alignment, and tighten all bolts to 30 ft. lbs.

To re-align the gas tank fill hose, loosen the pressure clamp behind the rear wheel well shroud and raise the hose about 1/2" to 1", then tighten:

Replace the black plastic cup that surrounds the gas filler tube, then replace your gas cap.

Presto!  After 300 miles, re-check the bolts.
